Chapter 3

Sienna POV

The acrid sting of rust and cheap bleach scraped at my sinuses like a file, dragging me back to a state of consciousness. My bare back was pressed against a hard, freezing metal surface.

I blinked against the harsh glare of a swinging bare bulb above me, my eyes slowly focusing on the blood-stained operating table where I lay.

Before I could so much as try to move, a brutal, tearing pain ripped through my lower body.

I screamed, but the sound was muffled at its source.

Someone had shoved a foul, chemical-soaked rag deep into my mouth, forcing it past my teeth and nearly making me choke.

Two men in dirty surgical scrubs were standing over me.

They were underground mob cleavers—the kind of back-alley butchers the syndicate used to extract bullets from soldiers who couldn't go to real hospitals.

One of them sneered down at me, his eyes glinting with a cold malice.

He called me a disposable whore who thought she could ride in luxury cars, but only deserved to be gutted in a dank basement.

He pressed a rusty medical instrument into my stomach with intentional, excessive force, twisting it slightly just to watch me writhe.

I thrashed against the table, hot tears streaming down my face.

It was futile; my wrists and ankles were strapped down tight with thick, unyielding leather belts.

The cleavers laughed, viciously insulting my mother and my bloodline to break my spirit.

They were maximizing my agony, administering it as a justified punishment for daring to think I was a real mafia wife.

Through the haze of blinding, white-hot pain, I heard a voice outside the heavy metal door.

A fresh chill coursed through me. It was Enzo.

His cold, authoritative tone echoed through the smudged glass window of the surgical room.

He ordered the men to hurry up and "extract the bastard" so he could take his real wife to lunch.

I turned my head weakly toward the window, my neck straining against the restraints.

Carmela was standing next to Enzo in the dimly lit hallway.

She was clinging to his arm, feigning tears with a sickening artistry.

She hit his chest lightly, her voice trembling as she claimed she never wanted to subject me to this brutality.

She whimpered that she only seduced him because she loved him, not to intentionally hurt me.

Enzo wrapped his massive arm around her waist and pulled her flush against his body, burying his face in her hair.

His voice dropped into a dark, possessive purr.

He told Carmela she was his one true Donna, and I was merely trash meant for the gutter.

He coldly warned her that he had no patience for loud, hysterical women, and ordered her to stop crying over garbage.

Carmela instantly dropped her fake tears, a triumphant smirk playing on her lips.

She stood on her tiptoes, her hands sliding smoothly up Enzo's neck.

She kissed him deeply, promising to entertain him properly tonight once this business was concluded.

I watched their passionate embrace through the dirty, blood-spattered glass.

My heart did not break in that moment—it was as if some vital, animating fluid had simply drained out of me, leaving a hollowed-out husk.

My body turned ice-cold, as if my very soul had evaporated.

The excruciating pain in my stomach vanished, replaced by a terrifying, hollow numbness.

As if in response to my surrender, the vital monitors next to the table suddenly started screaming in a thin, high-pitched wail.

One of the cleavers yelled in genuine panic.

He shouted that I was hemorrhaging massively and my blood pressure was bottoming out.

They dropped their bloody tools and rushed to the door, banging frantically on the glass to summon the Don.

But the dimly lit hallway was empty.

Enzo had already abandoned the premises with Carmela, leaving me to die in the dark.

The cleavers cursed loudly, terrified of having a dead body on their hands without the Don's explicit permission.

"We can't have her dying in here! The Don will have our heads if a corpse compromises this place!" one of them shouted.

They unbuckled the leather straps, roughly hauled my bleeding body off the table, and dragged me up a narrow flight of concrete stairs. The freezing night air hit my face as they tossed me into a damp, rat-infested alleyway behind the clinic.

They pulled out a disposable burner phone and dialed 911, screaming anonymously for neutral city paramedics to collect a dying woman in the alley, before sprinting back inside and locking the heavy steel door.

The next few minutes blurred into a chaotic haze of distant sirens and the harsh glare of flashing red lights spilling down the brick walls.

Legitimate paramedics burst into the darkened alleyway, carrying heavy trauma bags.

They dropped to their knees beside me, swearing under their breath at the absolute butchery they found on the wet asphalt.

A burly paramedic jumped into action, straddling me to perform frantic chest compressions.

A trauma nurse grabbed my blood-splattered belongings from a dirty metal tray they had tossed beside me.

She found my phone, unlocked it with my limp thumb, and dialed the emergency contact labeled Husband.

She put the phone on speaker and yelled into it, screaming that I was bleeding to death.

The call connected with a soft click.

Someone on the other end had fumbled with the device, accidentally hitting the accept button.

Instead of a voice, the unmistakable, wet sounds of Enzo and Carmela having violent, passionate sex broadcasted loudly through the echoing night air.

Carmela was moaning Enzo's name in a breathless rhythm.

Enzo was groaning, telling her in that same dark purr that she was the only woman he would ever truly claim.

A stunned, horrified silence fell over the entire ambulance crew.

The paramedic doing compressions paused for a fraction of a second, his hands hovering over my chest as he glanced down at the phone in sheer disgust.

My tears were gone now.

I felt absolutely nothing for the man on the other end of that line—not love, not hate, just a vast, echoing void.

I reached out with a weak, trembling, blood-covered hand and ended the call.

With the last ounce of my fading strength, I navigated to my mother's number and pressed dial.

The line picked up immediately, her warm, familiar voice answering.

I stared blankly at the starless sky, my voice nothing more than a hollow, raspy whisper.

"Mom... I was wrong."

"I know, baby," she said, and her voice was steel wrapped in velvet. "I'm already on my way. Stay alive. We're going to make them all pay."
